How to send and customize survey email notification after issue is been closed.

Hi @Fahad Siddiqui 

Are you using JIRA Service desk or JIRA software/core?

If it is service desk, the csat survey is a built in functionality - https://confluence.atlassian.com/confeval/jira-service-desk-evaluator-resources/jira-service-desk-customer-satisfaction-surveys-csat

 

And if it is software or core, you could try the commercial plugin  - https://marketplace.atlassian.com/apps/1213103/surveys-for-jira?hosting=server&tab=overview to see whether it fits your needs.

 

If you want something very casual and not much customized, you could modify the velocity template https://confluence.atlassian.com/adminjiraserver073/customizing-email-content-861253783.html?_ga=2.118800330.2110745071.1567475667-1686971808.1549273683 to send a custom notification for close events. In the email you could instruct users to perform an additional transition to fill up the questions you wanted to ask.
