Are you using JIRA Service desk or JIRA software/core?

If you want something very casual and not much customized, you could modify the velocity template https://confluence.atlassian.com/adminjiraserver073/customizing-email-content-861253783.html?_ga=2.118800330.2110745071.1567475667-1686971808.1549273683 to send a custom notification for close events. In the email you could instruct users to perform an additional transition to fill up the questions you wanted to ask.
