Insert new text rather than overwrite?

When I put the caret in the midst of existing text and paste or type new text, it overwrites existing text. How do I change to insert mode?

UPDATE: this happened only the first time I used the source editor. When I reopened it to show the problem to a colleague, it naturally worked as expected (inserting new text).

Let us know if you experience this again. If you could grab some screen shots and supply us with log files we may be able to figure out if there is a bug or if this was a one-off.

I'm on Chrome and I had this issue. It's definitely not my keyboard/browser. I pressed the insert key several times and couldn't get it to go away. I switched over to Gmail to compose an email and it wasn't in insert mode.

The only solution I could find was to logout of JIRA and log back in again. That seemed to make it go away.
