Create Jira Issue from Smart Checklist not working, but no error message on 'Create'

When creating a jira issue from a smart checklist item, the create button doesn't create the issue or provide an error message when it fails to create.

Is this for the App Smart Checklist?

If yes, and these instructions don't work, I'd recommend contacting the vendor for more direct assistance - to do this:

1. Go to their Marketplace App listing
2. Click on the Support tab - https://marketplace.atlassian.com/apps/1216451/smart-checklist-for-jira-pro?tab=support&hosting=datacenter
3. Select a contact option from the right-hand side
