I installed the add-on 'Email Helpdesk for JIRA'; and everything works just fine, except for one thing: when an existing ticket gets updated by the external user (external user sends an email to add some extra comments) then the comments are added in the ticket as it is supposed to do, but no notification is sent to the assignee of the ticket. (the external user gets a notification though)
Is there some configuration parameter I overlooked?

We are planning to add this option (send notifications on create / comment) on the next release 1.1, It will be released soon, I will update this answer when we release it.

The feature you requested has been implemented in the latest version of helpdesk for JIRA.
You can download version 1.7.1 (JIRA 7+) or 1.1.1 (JIRA 6.1-6.4), on the release notes you can find the feature described:
When the add-on creates a new issue the reporter and assignee will receive email notifications if the JIRA notification scheme assigned to the project allows it.
