Has anyone created user request intake forms for data projects using Jira?

Is there are quick solution to create intake forms for data projects that can be automatically converted to issues in Jira?

Hello @Rajiv Kolagani !
My team developed an add-on - Smart Jira Forms, which will allow you to share forms externally with users via link. The issue will be created after each form submission automatically and the form will be attached to issue. Alternatively you can export all responses in CSV.
