I have installed plugin version 1.0.1 without any errors in Jira 5.2.5.
After opening my issue i see new button with label "Error"
The issue has been fixed. Please upgrade to the latest 1.0.3.

I had the same issue. I use postgresql database. The reason there is an error is that they use a mixture of uppercase and lower cased column names causing case sensitivity issues due to "ISSUE_ID" column not existing because it's issue_id.
