Recommendations about create forms

Hi,

I would be happy to receive recommendations regarding Add-ons, whose job is to create form requirements that will eventually become a task in the Jira system.
The Add-ons you recommend will need to meet several criteria:

A. Create controls on the form, such as: CheckBok, Dropdown (which will show the list of projects in the system), Date picker.

B. An option to communicate on the document itself, if something is not understood.

C. Makes the form a task in the Jira system.
