Textfield 'as you type" function

Hi,

Following are the requirements i am looking for:
1. Textfield "as you type" function which should select a data from one excel form.
2. Once a user start typing they should be able to select that item, and hence should be able to add a comment in comment field and select , yes or no radio buttons.
3. Comment and radio buttons should reset to original once the information for first item has been done.
4. All steps should repeat for number of items, but each time the comment and radio buttons should be reset.

Please provide me a script , If it is possible and also, at which point to add this script.

What you've got here is a definition for a complex new field, not a script.  I think you'll need to write your own add-on to do this.
