Unable to import the test steps into JIRA using zephyr testcase importer

Hello,
We are trying to import several tests to Jira/Zephyr but even though the issues are created, the test steps are not.
We are using zfj-importer-utility-0.40.
We received the following error on importer utility:
Unable to add testcase ending at rowNumber - 5. Error:Unexpected response code: 404
<?xml version="1.0" encoding="UTF-8" standalone="yes"?><status><status-code>404</status-code><message>null
The excel structure we are trying (as a test) is the following:
