We have this same problem in Confluence 4.3.7 with Adaptivist Survey Plugin v 2.0.3. We have recently installed the latest Atlassian Security Patch and upgraded to Java JDK 1.7.0_25. Here is the error message we see when attempting to create a survey.

If we delete all existing surveys the plugin starts working again but this is not really an acceptable alternative. Also, we cant find all existing surveys unless we browse through every wiki space to find out whether or not a survey exists.

The problem is that there is a problem with the information in the database so none of the macros for the Survey Plugin work until we delete all existing Survey data from the Admin console.
