How to record\document the Approver Sign-Off when using DACI Decision?

We would like to have approvers sign-off documented on the DACI Decision page. The approver only gets an email notification that hew was mentioned on a page. It is not clear how he can provide her\his approval and it can be documented on the DACI Decision Page

Not sure if you ever found a satisfactory answer to this, but the approval could be done in a few ways:

1. Use an inline comment to add 'approved / not approved' as a comment

2. Write 'Approved / Not approved' next to the approvers name, then use bold or underline to confirm the decision

3. The outcome section could be used
