A member do an activity and the history is showing that another member do

Is it possible that a member performed an activity and the history is showing that another member performed that activity?

Is this a common mistake to happen? Has it ever happened to any of you?

Welcome to the community.

The only way I can think of is with a butler that will show the activities of the person who made the rules, not the one who executed then, but it does have a message warning that this is a Butler command.
