How to automatically create a Trello card from a Slack Workflow Form?

In short, what would be the best way to have the slack workflow with a form written by channel members, automatically generating a card on a Trello list? 

We have This one slack channel where users should be able to fill out a form and request a task to be done. Once he sends that in, we would like a Trello card to be created on our To-do-list. 

How can this best be achieved? Does it require Zapier?
