I want to create a Document on JIRA Hygiene

And the requirements are 

1) It should cover all the Issue type flow

2) When User Story are in Backlog what are the fields should present

3) When User Story are in Groomed what are the fields should present

4) When User Story are in Ready what are the fields should present

Each instance of Jira can be configured differently.  If you are creating a document then it will have to be unique to your instance and how it is configured.
