Confluence is a wiki - you add and update the body of pages as a whole.
It is not intended to be a form-based application, it's for free format pages, and the closest you can get off-the-shelf is to use templates (which still aren't forms, they're just pre-population of standard data)
There are a number of plugins which enable you to build forms though. Off the top of my head, https://marketplace.atlassian.com/plugins/com.frevvo.forms.confluence and http://wiki.customware.net/repository/display/SCAF/Scaffolding+Plugin
Will update the list by Nic created in 2013, as the time changes and new options become available: take a look at ConfiForms plugin for creating data entry forms and workflows.
