Is it possible to create JIRA issue from Confluence custom form ?

Can I create JIRA issue from Confluence page having custom form?

How can i achieve this using add-on or any hints that can configuring help.

Can i get confirmation back which JIRA issue id on confluence ?

Hi Jijo,

As far as I know, from confluence 5.4 onwards it is possible to create JIRA issues from confluence side even having custom fields.
