Can I customize the default "create issue" from Confluence to add additional fields?

Using the built-in functionality of Confluence and JIRA integration, we can easily create an issue from highlighted text while on a page.

Is there a way to customize the fields available on this form? I'd like to add "Assignee" primarily, but also would like to add "Component" (which may be trickier, as it is a multiple-select from a list). 

I found in the JIRA Integration plugin a module called "jira-create-issue-form" which I feel might be the thing to modify to add fields... 

Any advice is appreciated.

Hi Adam.

Long story short, the fields have to be set as "required" in Jira. After the fields are required in JIRA they will appear on the Create JIRA Issue dialogue in Confluence. Please see this article for an explanation and a link to the instructions for Jira: How to make a field visible in the Create JIRA Issue dialogue in Confluence

This is excellent, thank you. I searched hard for that article, but I wasn't able to come up with it, so I appreciate your pointer.

Unfortunately, this doesn't seem to work, or it has restrictions that are not defined on the page you linked. Was it incorporated only in a Confluence version after 6.5? 

Further info:
When I try to create an Issue with required fields, I get a warning that says "The Required Field name of field is not available in this dialog. You will need to create your issue in JIRA directly"
