Want to create form like a google form

I want to create a form like Google form provides and also want to include the attachment functionality which is not available in Google form created in shared drive.  I have gone through this link https://support.atlassian.com/jira-work-management/docs/how-to-create-edit-save-and-delete-a-form/ but not able to find in my projects or in confluence. Please suggest.

Forms could send emails, create Jira issues, create Confluence pages and lot of other things you can/want do in Confluence or Atlassian

It's not available in Confluence yet.

Navigate to your Jira Site and the forms are available in Jira Work Management projects as of now!!

I want to create a form which I can share in the organisation for particular request. I have tried with Google form but it has limitation and does not support attachment option when it is being created in shared drive. Hence seeking for option in Jira/confluence.

Is your project a JWM project? If so you should see the following in the sidebar. I suspect that your project is not JWM possibly it is a software project?

Yes I checked. It is software development project. Is there any alternate way to do it?

The new Proforma has been released/integrated into JSM project and is coming to Software projects eventually. For now you would need to consider an add-on application from the Marketplace .
