Hey, this is actually an old issue and we are observing this for several years at the moment. The issue is still there.
Date and time of transition shown differs in 'Transitions', 'History' and 'Activity' tabs. Real example:
- Transitions tab: John Doe executed transition - 2 days ago (tip shows 'Wednesday 11:27 AM'): Open -> In progress
- History tab: John Doe made changes - Yesterday (tip shows 'Yesterday 8:43 PM'): Status Open -> In Progress
- Activity tab: John Doe started progress on ... - Yesterday (tip shows '21/Jun/18 8:43 PM')
In fact, 'Activity' and 'History' shows proper transition time.
This issue makes 'Transitions' tab practically unusable while it could be super useful to check the transition history.

Hey, I'm using JIRA Cloud. We're using a regular way to transition tickets by using 'Workflow' dropdown transitions.
Transitions tab show inadequate data especially for issues that visit the same status more than once.
For those interested in a ready to use solution, Time in Status app by OBSS produces reports that show the first and the last status transition dates for each status. The app also includes reports that show how much time each issues spent in each status, assignee or group; number of times each status or transition was used.
Time in Status is available for both Jira Server and Jira Cloud (Date reports available only for Jira Cloud for the moment, will be coming soon to server.)
All report data can be exported as XLS, XLSX or CSV.
Hey Emre, I think this functionality should go out of the box with JIRA. Installing a separate app to track transition time is a bit odd, especially that there is already a history and activity which show this properly.
Btw, in the new ticket layout they've fully removed the ability to see transitions separately which is a shame.
