Can I use the new Forms feature with JIRA Product Discovery projects?
At this time, Jira Product Discovery (JPD) doesn’t support the native “Forms” feature like Jira Service Management does. So if you're trying to collect input or feedback for ideas, the native options are limited.
That said, you can use Smart Forms for Jira developed by my team as a workaround — and it works well with JPD in the following way:
Create structured forms to collect stakeholder feedback or idea submissions
Share those forms via public links, embed them in Confluence or websites — no Jira login required
When a form is submitted, a JPD idea can be automatically created based on the form response
All form responses are stored in the add-on’s Responses tab, where you can review, export, and analyze them

I understand you want to use Form on a JPD project. Please note currently, the Forms feature is not available for Jira Product Discovery projects. Forms are a feature of Jira Business Software and Service projects however not for Jira Product Discovery project.
Workaround: To collect ideas or feedback for a Jira Product Discovery project, you might consider creating a form in a Jira Business/Software/Service project and linking the issues/workitems with your Jira Product Discovery project issue/workitem.
