A form which is editable on JSM

Is this form created via a plugin/extension/off the shelf? How can I create this?

A lot of the Atlassian forms are custom and use the service desk API.  This is something that you can do if you have an inhouse developer.  There are a lot of great apps that would most likely meet your needs so you might check those out in the marketplace.
