Add a screen to a transition

We currently have a workflow transition that allows us to "Escalate" a ticket.  While this function works, we now want to add a screen that requests a few pieces of information.  I have the fields I want to use, but how do I add that screen to come up when it is selected from our Workflow drop down list?

The transition works just fine, we just want to trigger a screen asking for fields to be added.

Hi,

By default, in native Jira Service Desk, you can't display any transition screens on the Customer Portal. You can achieve only using the app Actions for Jira Service Desk. It allows to display system and custom fields on the transitions screen for customers.

Please not that if the individual that is escalating is a customer they will not get a screen. Only agents or collaborators would see a screen within the app.
