Add fields completed in transition screen in view/edit

Hello,

I have an issue and I cannot figure out how I can implement the following:

Reproduction steps:

1. User will create an issue 

2. User will change issue status 

3. While changing issue status, a transition screen with some mandatory fields (add some text) - user will complete them 

4. After user typed the necessary info into the fields and ticket is in the next status -> show them into the ticket 

Can someone advise me how can I implement step 4 ?
