I follow the destructions, created a form and added a number of custom fields to be filled in on the form. However, once the form's filled in, whilst a ticket is created, it doesn't contain any of the data from the custom fields on the form.
Permission-wise I've given create issue and browse permissions to the intake system user.
I was under the impression that there is no need to create actual fields in Jira for the data to be recorded into the ticket, using this product.

We use this plugin in our company and I am pretty sure only the fields that are in Jira can be saved through Intake form.
As this plugin does not have own storage and can only store the data inside the Jira ticket it creates. So, you need to have (actual) fields defined in the project in Jira
