Case:
1. user starts from a link to create issue with init details, such as
https://<jira_base_url>/secure/CreateIssueDetails!init.jspa?pid=12345&issuetype=7
2. clicks on attachment icon in the description toolbar
3. attachment icon disappears
Note that: if you check the html code, Attachment button is still there, but something in the css hides it.
If you remove the class attribute it appears again.
