Can I install Jira service desk as a stand alone product or do I need to install Jira first?
Yes, you need a Jira isntallation to run Service Desk on.
However, if you download Jira service desk from the site, it is a bundle of Jira and Service Desk, so you only need to install that to get both in one go.
Great, we want to test it out/kick the tires to see if it will work for our IT team help desk request. We just initially purchase a 3 agent version. Is there much customization needed from what is "out-of-the-box"? And do we need to install a database first or is a database bundle with Service desk too? So I just run the downloaded Jira Service desk installer and good to go?

The installer will ask you what database you want to set it up with. It can use a file based one that the installer will offer, but this is not suitable for production use - it does not scale well and it fails catastrophically quite a lot. It's for testing only.
You will need to create an empty database for it, and give it access, but the installer will create everything it needs inside it.
