Per your description, I understand you would like to add a select list field to the resolution screen of your issues that will list the three pre-determined options you mentioned. Is that correct?
It's quite easy to create a select list field with the pre-determined options you need and add it to the final screen or your workflow. These would be the steps:
That being said, I noticed that you want to add the reference to the new request if the user resolving the issue selects the option "Further Action Request raised" or "Non-conformance request raised". This information can not be added to the pre-determined list of fields automatically, so I believe you will need to use another text type custom field to add this extra information.
