Create a Service Desk Ticket request with Yes/No answers

I need to create a service desk ticket request that contains yes/no branching. Like this.

Reset FunkyWidget Password

Are you using a Windows Laptop?

Yes|---> go to the FunkyWidgetPortal and choose reset. (End)

No |---> Connect to VPN and go to MacFunkyWidgetPortal

Were you able to go MacFunkyWidgetPortal?

Yes|---> Choose MacReset (End)

No |---> Ping IloveRyanSeacrest.com

Were you able to ping IloveRyanSeacrest.com

Yes |---> Email to FWP support.

No |---> Create ticket and schedule to bring in your laptop to the IT lab.

 

Is there a way in Jira Service Desk Cloud use Yes/No answers to capture answers and send the user entering the ticket down different branches based on the answers?

If this would require an add-on which do you use that would do this?

A great feature called Dynamic Fields for request types

Based on your previous selection new field will revel, you can test it for free.
