Good morning all!
We are running into multiple issues while using Proforma form in SD Help Center (HC) request.
First of all: The form itself displays all the correct fields etc. The issues begin right after creating a request/ticket from HC. We are using "service request with approval" type. This request/ticket flow works fine without using Proforma Lite form (tested).
First issue:
Supervisor receives e-mail to approve the request. He/she clicks the "View request" link in e-mail and is being forwarded to the request details. After pressing "approve" button, Jira gives out a error with lots of rows, first row: com.atlassian.jira.index.IndexingFailureException: Indexing completed with 1 errors
Second issue:
We go to the created task detailed view in SD environment. After we press "approve" inside the task, we again receive the error com.atlassian.jira.index.IndexingFailureException: Indexing completed with 1 errors, after we refresh the request, the request is approved, BUT status is unchanged, it is still in "waiting for approval status".
So basically, for conclusion: we are receiving lots of errors when trying to approve. Statuses are unchanged when doing some activity. Seems like this (Proforma lite form & JIRA SD) does not work along well or are we doing something wrong? Wrong workflow or something? We are stuck and maybe somebody could help us out?

Hi Kaur, I’m the product manager for ProForma. We are happy to help anyone who uses ProForma regardless of whether you use lite or full. You can create a request via our support portal.
As to the specific error you are seeing, this is something new and the dev team will need to take a look at this in more detail.
Continuing this via a support request will probably be fastest, but I’ll definitely post an update here on what we find.
