Is it possible to ensure that attachments made as part of internal comments are not visible to service portal users.

Thank you for reaching out to Atlassian Community!
When adding an internal comment, any content added will not be visible in the customer portal.
Even if an agent or an admin views the ticket in the portal, they won't be able to see the internal comments, it will only be visible internally.
