How do we attach a file/document to a response from Jira Service desk?

Those new to the Atlassian Community have posted less than three times. Give them a warm welcome!

When I respond and attach a file, clients do not see the file, just the link. Requests come in via email.

You can send files as attachments using our add-on Raley Email Notifications

That is how it works. Emails only provide links and customer must go to portal to view. You might find this thread worth reading.
