Automation - update description with a custom value

Hi, 

I'm trying to see if there is a way to automate the update of the description with help of a custom field we have. We have a field that we type in the Change Request designation so when that field is populated (should only be done once). 

the new description should be something like:

custom field - {old value retained}

anyone get any ideas?

I used the "Change type" custom field as an example, but you could of course replace it with the custom field you need :)
