Thanks. But I want to add the standard attachment in portal so that user can fill it while sending the ticket.

In the help and instructions section of the request type you could add a link that would allow them to download the document, fill it out and then attach it to their request.  You can use [link name|http://example.com]

Sorry don't understand what you mean by "standard attachment". Here is an example of what including an attachment looks like in the portal. If that doesn't work please elaborate. Regardless this is what JSD provides.

I want to keep word document / form in the portal. they need to fill and send it as attachment with the ticket.

user will not have the standard document. so they need to download and while sending they need to fill it out.

This is not possible. You might want to look at the Marketplace to see if there is an addon but not familiar with one.

You can do it using Confluence and Jira service desk, by enabling a space as a knowledge base.
Then, in the knowledge base space, you can attach a file in an article (page)
Then you go to "attachments" in that page page an take the URL of the file.

Then you use that URL in any description of the request type, or in the banner, or wherever you want.
If you are using Proforma, even easier.

And even more cooler,  if you have scriptrunner (data center) you can use fragments and use it anywhere.

Whoever has access to the portal, they will have access to that URL , since Knowledge base and Confluence has this special permission relationship.

Cheers!
