We need a single request type to be laid out like this:
The first row (with "Sick Remaining," "Vacation Remaining," and "Comp Remaining") is for the employee's information only; these fields are read-only.
The fields in the second row are all related, the fields in the third row are all related, etc.
Also, how would the data in the issue look in the issue when the agents work on the issue? Could it be presented similarly?

You can not change that much Jira UI. If you want to have your own UI, you could write your own application and use Jira Rest Api to update data in Jira.
