¿Es posible agregar un formulario personalizado para las solicitudes de jira service desk?

¿Quiere decir "adjuntar" un formulario personalizado o simplemente construir el formulario personalizado en la solicitud de creación? Lo último es precisamente cómo funciona JSD.

debe crear su formulario simplemente agregando campos y haciéndolos requeridos u opcionales. Haga esto por Tipo de Solicitud. Añadir un archivo adjunto sería un proceso manual, creo.

Hola, cree dos formularios uno para solicitudes y otro para incidencias pero no se como configurarlo para que funcionen cuando cree una solicitud o insidencia, espero puedan ayudarme.
