Custom Fields - list of check boxes

In a service request with approval, I want to add a custom field that is a list of checkboxes associated with applications a customer requests access to?  How?

There is a custom field called "Checkboxes"

Thanks. I eventually found the custom field I needed and created the appropriate field for the request.

not sure about "checkboxes" but you could consider a multiselect list.
