When an issue is created I setup a security group for confidentiality.
The current assignee need to add a watcher of their group that is in the same security group.

The permission setup is controlled by your JSM project's permission scheme. You just need to use Project settings > Permissions, then click on the Actions button > edit permission. Make sure the current assignee are granted with "Manage Watchers" permission.
NOTE - In JSM, you should use "Requested Participant" concept instead of "Watcher" since typically only agent can access JSM project issues directly via the project UI. All other users are accessing the issue via Portal UI.

The current assignee can add a watcher if they have maintain watcher permission. They will be limited to selecting from the people who can see the issue.
