As Nic said above, ProForma: Ethics Approval Workflow is a workflow bundle rather than an app. It that can be used for importing into your Project as a workflow. Is this something you're interested in using?

Are you on Server, DC or Cloud?

The JWB means it is a bundle of workflows, not an app.  You'll need to follow the instructions for installing it, not just try uploading a copy as an app.
