Hello,
For Service Desk request forms we have created custom fields and set a default for those custom fields. All of the forms show up and work correctly, but when viewing some of the issues in the issue queue the fields show up (with the set default value) that are not questions on that form.
How do I stop this from happening?

You can try removing these fields from the Screen configuration.
