Ask questions before creating the issue

I would like ask if any chance i can ask 3 random question to the users and after answering correctly ( i mean yes or no) then incident form would appear on the screen and complete the incident form. Otherwise just finish the questioner withour showing the incident form.

I know i can include this questions inside the incident(automation check custom fields) but i would like to have it separately.

I don't believe any Jira automation or script can be invoked at that point. I suspect you would need a webpage in front of Jira to ask the question and then allow them to enter the issue. Is this for security purposes?
