I want to use Microsoft Flow to create a Jira issue from a Microsoft Form submission.
To set this up I need to add a couple of things including the URL of my Jira instance. I've tried the URL that I use to access Jira at work, but the connection fails.
So what then is the URL of our Jira instance? Is there another URL for the Jira instance that is only known to admins?
The Jira version I'm using is server 7.11.
No, the url it is asking for is the url you visit in your browser. Can you reach the url you use at work in a browser?

Yes, our Jira instance is accessible in the browser, but the URL is strange as it's in this format alpha:1234.

That's a url with a port specified on the end of it. That should not be a problem. Is the system running https? What is the error you are seeing when the connection fails?

I'm getting the error message 'Please check your account info and/or permissions and try again'. One thing I've noticed is the URL is http:// which I imagine is an issue.

Jira URL in my company is https://jira.companyinitials.ad/
When use the above URL and my username and password to access Jira it doesn't work.
Does it matter what I put in Connection name? I'm adding CompanyNameJira

I found the problem which may be helpful for many people here.
1. Microsoft Forms is a cloud application it is hosted outside the organization's network.
So for Microsoft Forms to connect to your Jira your network administrator has to make configuration changes in network firewall to allow Microsoft Forms servers to talk to Jira servers hosted in your organization's network.
My organization won't allow that kind of network communication so it won't work even if the username password and server details are right. Microsoft should elaborate their response saying what is exact connection problem is it communication problem with server or user authentication problem. Current Connection Failed response/error sucks!!!

That is a very good point when I read back. The conversation here really does concentrate on "put the right data in", but does not mention exactly what you say - there may be network blocks as well.
You're absolutely right, Jira has no way to know that the failure to connect is network or data related because Mcrosoft have chosen to give us useless errors.
I am afraid I can only "like" your answer once, it deserves a lot more.

Where do I generate a Token API at Jira Server (Not Clould version ?)

Unfortunately, there are no ability to do it w/o 3rd party plugin.
But you can use API Tokens for Jira to create tokens and keep personal password in secret. Available also for Data Center and Confluence.

Version of the Jira,
Version of the API Tokens plugin,
The most important, do you have Crowd SSO (or any other) connected,
Ideally, logs from Jira server with Debug level of logging for com.wombatscorp

This is usually solved by giving the account you are using the right access in Jira and giving the form the right username and password to Jira.

I can't tell you anything more than what has happened above - you need to give Flow the right connection data.

@Nic Brough -Adaptavist- can you please tell me Which are the right account permission to connect Flow to Jira? because my flow working fine for one month but suddenly stop working and give this error "Please check your account info and/or permissions and try again". when i connect JIRA in browser with same credentials its working absolutely fine.

The only way I could get it to work was to use the API token.

Connection Name: JIRA
JIRA Instance: http://companyname.atlassian.net/ (with or without the trailing "/")
User Name: my users name
API Token: the token I created in JIRA Admin
