Create repeat issues

When I create one issue, but JIRA generate multiple issues at the same time.

it has occurred twice. Firstly,  JIRA create 4 issues , but i only click one 'Create' ; Secondly, JIRA create 2 issues, and i only click one 'Create'.

If you haven't worked out what's happening and JIRA is creating erroneous issues, I'd suggest contacting our Support team.
