Custom Automation Rule Help

I've been using Automation Lite to set up multiple things for our Servicedesk, but am still struggling trying to make one of them happen. My last post:

Details:

2 Custom fields - Impact, with options 1-4. Urgency, with options 1-4.

When these fields are changed, I want them to change Priority (options 1-4) based on a simple 4x4 matrices. 

I have tried multiple setups, including those suggested by Alexey, who has been quite helpful, but to no avail. At one point all rules were firing at once, now I can't get them to go through at all. 

I would like to start from scratch and follow some basic instruction from someone with experience in Automation Lite. 

Again, upon editing Impact and Urgency, I would like Priority to auto-update following a 4x4 matrix.
