Force user to enter a comment on workflow transition

When moving an issue from one state to another, I would like to force the user to add a comment to the issue and then allow the transition. How can I do this please? I have already tried the Add Required Field validator.

To summarize you need to

1. create a screen with a comment field
2. add this screen to the transition
3. and make the comment mandatory using a Required Field validator.
