I am trying to setup a service desk and want to be able to provide custom drop-down options to pick from and provide mini description of what it means. Just like the below screenshot that I took from Atlassian support form itself, but can not reproduce it. What am I missing?

You might want to try adding a Message type custom field to your screen right above or below the drop-down field that has the explanations of what each one is. But you aren't going to be able to add values like what you have shown.
While you can certainly create a single select drop-down it does not provide an option to include a description. The best option would be to ensure that the selection itself is as self-explanatory as possible.
