How to create a field to collect the rating for 5 items within a single field

How can we create a field so we can collect the rating for 5 items within a single field using Jira, JMCF, or Scriptrunner? We do not want to create 5 fields plus a 6th field for the total nor do we want to create a mult-select field with all the combined options.  

Users would select High Med Low for each option, which on the back end would have a numeric value. High = 3, Med = 2, Low = 1

Example:

New Field: Rating Custom field

Option A: High
Option B: Med
Option C: Low
Option D: Med
Option E: High

Total: 11

Unfortunately there is no native solution available for this, apart from using individual fields for each option.

This plugin provides an option for Table Grid which can be used as a single field with multiple rows. The other option is multi level cascading field. Both of these option can be used for your requirement.
