Jira Service Desk - Send a private form to customer

Hi,

At the moment we need some confidential information from our customers, and we send them a PDF which they need to complete, but as he switches to Jira Service Desk now, is there a way to send a form into a ticket or by mail to customers which they can complete?

There will be sensitive information in that form - im looking at a way they can see it just once....after they close the tab or something...the link will not be available anymore.
