I created a rule that is triggered by a task transitioning from In Review > Done. The rue checks the following conditions:
And if those conditions are met I do an action: Transition the issue to and I select In Review and post a comment that the initiator needs to get legal to review and external documents.
I ran a test where the conditions were met and the rule was not triggered. Am I doing something wrong in the set up?
Going to leave the solution just in case someone else runs into a similar issue.
The reason why this rule was not working is because I did not have a transition from Done back to In Review when the conditions I had in place failed. Once I added it the rule worked.
HI, Steve, I see you already said you were going to engage support but I'm a bit too curious for my own good :)
Make sure the project you're running your tests is in the scope of the Rule. At times we create the rule in the wrong project and it gets confusing why it's not working.
Also, try editing the rule (assuming this is a test project!) to run whenever it goes from any status into Done.

Can you please provide screen images that shows the complete automation rule and the details of each step?
Also can you provide the details of an issue that you transitioned from Review to Done that you believe did not trigger the rule?
And lastly, can you check the Audit Log for the rule to confirm that the rule actually was not triggered, vs. triggered but failed or triggered but no actions taken.

Was the issue in the In Review status and transitioned from there to Done after you created the rule? As that is the trigger condition and the rule was not triggered, that would suggest that you did not make that exact transition.
Or is the rule not turned on?

You cannot get to Done status without going to Review first, our workflow doesn't allow any other path to Done.

Is this a Team Managed project or a Company Managed project?
Can you show us the workflow diagram showing the status transitions?
Can you show us the History for this issue that shows the transition from In Review to Done?
Have you tried transitioning another issue in the same way to see if you get the same result?

I am stumped.
I have created a simple Automation rule in a test Team Managed project where the rule is triggered by a transition between statuses where there is only one path, like yours.
I see an entry in the Audit Log for the rule getting triggered when I make that transition. And there is not entry for other transitions because, of course, the rule is not triggered in those cases.
I recommend that you contact Atlassian Support directly on this one.
