Bulk CSV import not working

Below is my CSV file nothing is coming into Jira, please help me in identifying my mistakes:

Summary      Issue type         Story points       Issue ID       Parent ID         Subtask type

Collab UX       Feature                                           1              

WAH BSC         story                     3                       2                 3                      story

WAH deploy     Feature                                         3 

Collab dash       story                     5                      4                 1                      story

I do have a few more columns. but these were important, what am I doing wrong?
