How to add a custom field in customer portal?

I guessed you are using JIRA Service Desk cloud, so the links below contain documentation for Cloud. The documentation for Server might be a bit different.
You can configure the request type as described on this knowledge base article.
However not all types of fields are supported, please check the list of Unsupported fields.
